Direct Support Worker 

Job Summary

 Inaadiziwin – “A Way of life” Supported Independent Living to youth supported by various Child and Family Service Agencies. We provide a culturally based environment where relationships and attachments are prioritized, and physical, mental, emotional, and spiritual needs are nurtured.

The Community Support Worker (CSW) provides support to adolescents in a Community setting and is responsible for ensuring that the participant’s physical, emotional, social, educational, and medical needs are met. The CSW will also assist participants enhance their quality of life with activities of daily living and the development of life skills. The CSW is responsible for the delivery of quality individualized support and fostering a safe and comfortable home environment.

Accountable to:  Program Director/Coordinator

 Qualifications

Education and Requirements

  • Grade 12 Diploma or equivalent;

  • Post Secondary Diploma/Certificate in Community Support Work, Indigenous studies, Mental health/Disability, Addictions, Social services or related field; or a suitable combination of education and experience;

·      Current First Aid/CPR;

·      Nonviolent Crisis Intervention training;

·      Clear criminal Record Check, Adult Abuse Registry Check, Child Abuse Registry Check;

·      Valid Class 5 Driver’s License, and access to a Vehicle.

Experience

  • Experience working with the vulnerable sector, including CFS, CLDS, physical, mental, emotional and behavioral challenges and difficulties;

·      Experience working with people struggling with addictions;

·      An understanding of people who are marginalized is considered an asset;

·      Experience in the development of behavioural plans, risk management (organizational and participant related) and the person-centered approach;

·      Knowledge of behaviour modification, supportive communication, and the stages of change;

·      Experience and sound knowledge of historical impacts of colonization and Indigenous culture; knowledge and experience working with the 7 teachings would be considered an asset;

·      Strong problem identification and usage of culturally sensitive approaches to solving issues including conflict resolution;

·      Self motivated with an ability to work independently;

·      Computer skills including Microsoft Office suite;

·      Extensive and diverse experience as a volunteer is an asset.

Duties of the position

·       Assist in support services and ensure program objectives and individual support plans are delivered, developed and maintained according to organizational standards;

·       Participate in the amenability/intake/transition process of new participants to the program;

·      Meet with the participant and collateral's/stakeholders prior to the onset of service to gather information about the individual to formulate a support case plan;

·      Participate in the development of an individualized profile and support plan for the participant;

·      In collaboration with support team, address needs pertaining to participant transition as necessary;

·      Employ a person-centered approach

·      In conjunction with support team, engage participant in a process of goal setting, planning and acquisition as it pertains to living safely and independently in the community;

  • Coordinate activities on a week-to-week basis to ensure a balance of personal and social opportunities to promote recreation, community interaction and individual relationships;

·      Exploring recreational, educational and employment opportunities, etc.

·      Exploring and accompanying participants in the program to cultural activities and ceremonies;

·      Assist with transportation to and from activities as required;

·      Teaching and supporting independent living skills such as: household management, personal hygiene, cleaning, cooking, budgeting, and grocery shopping;

·      Assist with securing housing and assisting with the moving and transitioning process;

·      Provide leadership and serve as a positive role model, encouraging healthy relationships, communication, and coping skills;

·      Assist participants in gaining work experience with our social enterprise Operation No More Snow property maintenance. (Lawn care and Snow removal)

·      Report at-risk participant activities;

·      Perform personal care duties including storage and administration of medications as required;

·      Oversee and maintain an awareness of cumulative health care of the participant (including general practitioner, dentist, ear and eye specialists, and psychiatrist appointments)

·      Attend medical appointments, critical probation appointments, supervision meetings, planning meetings and all other necessary appointments’.

 

Reporting: 

·      Complete and document training for all program participants;

·      Adhere to the reporting expectations of the funding body for each individual within case load;

·      Submit formal standardized written incident reports following an incident with the participant;

·      Respond to crisis/emergencies as required;

·      Maintain up-to-date files on participants;

·      Acquire monthly reports and staff logs;

·      Perform other duties as assigned.

 

Salary Wage: 41,600 – 52,000 a year.
